Bullous Pemphigoid is an autoimmune disorder of the skin. Pemphigus is a group of skin disorders of which Bullous Pemphigoid is one type.

This disease is generally benign and characterized by bullae or vesicles (blisters) of the skin which tend to be very pruritic (i.e., they itch). Oftentimes, due to excess scratching, the skin becomes secondarily infected with bacteria requiring antibiotic treatment.

It is caused by antibodies directed against the basement membrane of the dermis or skin, hence the term autoimmune. Inflammation, due to a cascade of immune modulated events, develops in the dermis resulting in blistering of the skin, increased tension and intense itching. The fluid in the vesicles or blisters is serum. It is sterile.

Aside from its inherent cause of discomfort the disorder is sometimes associated with other autoimmune diseases such as Systemic Lupus Erythematosis. It has also been associated with certain cancers.

Bullous pemphigoid is diagnosed based on the results of a skin biopsy, which when specially treated can demonstrate the presence of antibodies deposited in the basement membrane layer of skin.

Bullous pemphigoid is generally mild but chronic, not significantly affecting the general health of the person. Treatment, first line, is with topical cortisone creams, but depending on the severity of the presentation may require high doses of corticosteroids taken by mouth.

Severe bullous pemphigoid may require immunosuppressant drugs, such as azathioprine (Imuran) or intravenous immunoglobulin treatment, generally given on a monthly basis.

The majority of cases occur in persons 80 years of age or older.
