How do you make a Formal Complaint agianst a dentist?!

How do you make a Formal Complaint agianst a dentist?
I have already looked into the ada.org but i can't seem to find where to make a formal complaint.
Answers:
The ADA has nothing to do with accepting complaints, but the state components do have dispute resolution boards.
If you want to file a formal complaint, you have to do this with the "State Board of Dentistry" in the state where the offense was committed.
