Dental claim form?!


Question: Dental claim form!?
My insurance policy says that if expenses are over $300, i should notify them before treatment!. How do you go about doing this!? Do i have to go to my dentist and tell him to give me an estimate without consenting to the procedure that i need done!? Do dentists give estimates of the costs even if you don't plan on letting them do the procedure!?Www@Answer-Health@Com

Hopefully the dental office has already done fhis for you!. They will submit a pre-treatment estimate, which is a list of treatment needed, to your insurance company with copies of the xrays to 'prove' you need the work done!. Their fees will be included on the form!.

Once they receive it back (and you should get a copy as well) you can go ahead and have your dental work completed, and you'll know what your out-of-pocket expenses are since the insurance company will have what they will pay right on the form!.

There is no charge for them to send in the pre-treatment estimate!. Keep in mind, too, that it is an estimate and not a guarantee of payment!. Deductibles and annual maximums are taken into consideration!.

If your annual maximum is $1000, that doesn't mean your insurance company will pay that amount for a root canal, for example!. A percentage of it will be paid, leaving the rest of your annual max for other procedures!.
