The answer to this question all depends on your specific situation. Are your wisdom teeth impacted? If so, then you will have to go to an oral surgeon and they will give you general anesthesia (put you out) and they will more then likely take all four out at one time since it is very expensive to be put under, and paying twice and healing twice would be pointless and the risks would outweigh the benefits.
If your wisdom teeth are not impacted, then a general dentist could probably do it, and you would probably just need a local anesthetic (just blocking the nerve in that area) and possibly something to relax you (ie. Valium). If you went this route then the choice of having 2 out or 4 out would be more up to you and your dentist. Most dentist's will recommend having all four out in one single operation, but sometimes people do not need all four out and live their entire lives with wisdom teeth.
What I am saying is, you should have a talk with your dentist. He is the only one that knows your specific situation, and, in turn, is the only person who can answer these questions for sure.

TAke your dentist's advice and if necessary, go for a 2nd opinion. Try to avoid general anesthesia, that has it's own risks. If you do the general anesthesia route, you should be sure you do it somewhere where there is a professional anesthesiologist.
And remember general anesthesia will add to the cost and to the recovery time.
