as a matter of fact there was a company and their insurance co. that caused a big stir not too long ago because they were. they didn't allow smoking at the facility because the health ins. provider didn't want to cover smokers, and they were testing the employees when thet were sent for physicals.

Not sure,but they could tell by smelling your clothes or hair.
