Answers:
Yes. It's called aspiration of gastric contents, and it still kills people who violate the order not to eat or drink, or those who have stomach emptying problems, or have to have emergency surgery right after eating.
There's no guarantee that you'll die, just an increased chance of it. Food in the lungs is a bad thing.
If you've eaten and still need the surgery, we can put a tube through your nose, into your stomach (while you are wide awake) to suck out all the food and liquid. Sounds like fun, doesn't it?
Don't eat or drink before surgery.

if you eat or drink before an op, while under anaesthetic, and while the tube is down your throat, you can vomit and it can get into your lungs.
if you have food in your digestive system and you have an op on your stomach, it can lead to infection.
