If it's painful it could be a basic pimple (yes, you can get 'em there too) or a cold sore. Usually it's pretty easy to tell the difference, a pimple will form a head in a few days, use a bonzoyl peroxide cream, fix it right up. If it's a cold sore, it'll be really sore and it'll last longer, try an over-the-counter lip medication designed for cold sores, you can find 'em everywhere.
If you don't think it's either of these, or the medications don't work, try asking the pharmacist at your local Walgreens, WalMart, CVS, etc. and they'll be able to give you apretty good idea of what it is and if you need to seek medical attention.
