Yes, several times:
1) When I was 3 i jumped off a chair onto a cushion, which promptly slid across the shiny tiles with me still on it!.!.!.I fell over backwards and cracked the back of my head on the marble tiles!.!.!.out for about 30 seconds!. When I came round I had the most horrible feeling, as if the world was ending!. I screamed and screamed my head off!.
2) Used to play at standing on a swing, getting as high up as possible then jumping off!. This time I was about 5!. Unfortunately it was one of those swings that has a sort of safety cage to hold a small child in so, when I jumped off, my feet became all tangled up and instead of flying off upright and landing on my feet I actually went flying down head first onto the concrete!.
Went out cold, woke up with a huge crowd around me then my forehead literally swole up like a balloon!.
Swelling so severe I had to have immediate Xrays to check my skull wasn't fractured!. I still have a dent in the middle of my forehead!.
3)When I was about 12, I had a fight at school with 2 boys at once (cowards), and one of them punched me square in the face and i went out like a light!.!.!.!.the headache for the following two days was unreal!.
Oh and I passed out one other time but this time it was voluntary:
At a school I was at when I was about 10, we played a (dangerous) game!. You had to crouch down by a wall and do heavy, rapid breathing, about 50 times, until you were feeling dizzy and light-headed!. Then you had to stand up really quickly and immediately about 6 kids pushed on your chest (your back against the wall) with all their strength and you passed out cold!.!.!.I don't know how long I was out for but during that time I had one of the nicest dreams I've ever had!.
Unfortunately one of the other kids must have had a godawful experience because he woke up screaming and crying his eyes out!.!.!.don't try it at home!!!!
I had so many other very serious knocks to the head as a child that I honestly don't know how I'm not brain damaged!.!.!.!.!.or maybe I am, LOL!!!Www@Answer-Health@Com

When I was a kid (14/15 years old) goalkeeping and Sean P kicked me in the head as I took the ball from his feet!. Out for one or two minutes!. Spent the night in Perth Royal Infirmary for 'observation' and fainted in class the day after that because I had to stand up for not doing my homework!. Ah sweet memories!.Www@Answer-Health@Com
