Answers:
It is a highly innervated area, very sensitive to pain!.

The pain radiates up the abdomen and to the flanks of the back due to where the nerves supplying the testes actually originated!.

You may be familiar with the fact that the testes descend into the scrotum during the first few months of life!. In utero, the testes actually sit just below the kidneys and gradually grow down through the inguinal rings to descend into the scrotum at birth/infancy!. On the journey down, the blood and nervous supply is dragged down too, creating the spermatic cord which you would be able to feel at the top of each testis!.

So an injury to a testis will send the pain sensation right back to the origin, up through the abdomen through to the back near the kidneys!. Www@Answer-Health@Com
