Depends where you are. I'm Canadian, and all you need is cash or your provincial health card. I did an internship in the US, and they didn't require a SSN from what I can recall.
Essentially we need your personal info (name, date of birth, address, phone) and a method of payment. If you're having the government or an insurance company pay for you, you need to bring the appropriate information along (government health card, insurance certificate card thing, etc).

I'm an optometrist.

What country are you in? In Canada (where I am) you need your health card and some form of identification to see an optometrist.
