If you want to get really technical about it, it's a reflex response to nerve signals.
It is ultimately the brain that interprets signals (which can come from any part of the body, even the imagination, to cause an orgasm); but it's usually three sacral nerves -- S-2, S-3, and S-4 -- that carry nerve impulses to and from the genitals that provide the sensations that result in an orgasm.
Every time one of the many nerve endings in the genital region is stimulated (usually by some sort of friction or pressure), a signal travels through the sacral nerves; part of the signal goes up the spinal cord to the brain, where you "feel" the sensation, while another part of the signal turns around at the spinal cord and goes back to the genitals and causes more sensitivity, lubrication, etc. This cycle keeps going until the cummulative effect of the nerve signals finally reaches a level where your brain and spinal cord react by causing involuntary muscle contractions in the genitals and other places in the body, which are very pleasurable. (Hormones, thoughts, and other factors besides just stimulating the sacral nerves also play a big role in how the brain ultimately responds to the nerve signals).
Like I said, it's a reflex, just like when the doctor hits your knee with a hammer and your leg jumps -- only instead of one tap to set it off, it takes a lot of friction, and instead of one muscle contracting once, many muscles do so repeatedly.
The other reflex that's closest to an orgasm is a sneeze, where irritation of the lining of the nose stimulates nerves more and more until finally many muscles contract forcefully, sometimes repeatedly, in response.
