It is possible - remember tampons will always leave fibres behind in your vagina, although tampon manufacturers carry out batch tests to limit fibre loss these safety tests are not independent so are of questionable reliability and it is still possible for larger amounts of fibres to be lost.

If you don't want to get TSS then don't use tampons, try menstrual cups or softcups, as well as no risk of TSS or infections they're more convenient as they can be worn for 12 hours with any flow, overnight and before your period, menstrual cups don't leak like tampons and can lessen cramps and flow in comparison too. As well as expanding in water they don't break down so block pipes further up the system which costs billions a year to fix, also they can make it through the sewage system into oceans. Some tampon companies claim their products are flushable because they're self-regulated so can make this claim despite environmental risks - always dispose of tampons correctly by throwing them in the bin. That's the normal shape of the tampon. Tampons go through a series of tests when being made to ensure that this doesn't happen.
It's just shaped like that when it inflates, don't worry! :) Tampons can not rip inside you!
